Membership Agreement This Membership Agreement ("Agreement") is made between Evergreen Play Club ("Provider") and the undersigned member ("Member"). 1. Offer Details: Membership Type: By committing to a 3-month membership (June, July, August), the Member will receive a 50% discount on the first month of their membership. Commitment: A commitment to a 3-month membership is required to enjoy this discount. 2. Membership Rates: Individual Membership: First Month: $22.50 (regularly $45) Second Month: $30 Third Month: $30 Family Membership for Two Kids: First Month: $32.50 (regularly $65) Second Month: $50 Third Month: $50 Additional Kids: First Month: $10 for each additional child (regularly $20) Second Month: $15 for each additional child Third Month: $15 for each additional child 3. Payment Terms: A valid credit card must be kept on file for the duration of the membership. Membership fees will be charged as follows: The first month’s fee will be charged upon signing this Agreement. Subsequent months will be charged on the same date each month thereafter. If a payment cannot be processed on the due date, or within 3 days after, a late fee of $10 will be applied to the account. Should the late fee and membership payment not be received, the amount due will be sent to collections/small claims court. 4. Membership Duration: This membership is for a total of 3 months. The membership will automatically renew at the regular rates unless canceled by the Member prior to the end of the membership period. 5. Cancellation: To cancel the membership, the Member must provide written notice to the Provider at least 7 days before the end of the current membership period. 6. Acceptance: By filling out this form, the Member agrees to the terms of this Agreement
